The location of the CPT Guidlines on the CPT Bookshelf

The GUIDE section contains the following four items:

-CPT Guidelines which are the AMA's instructions for use of the CPT codes in general, plus use of CPT codes within each of the six sections*:

-CPT Modifiers may be attached to codes when they are billed in order to provide added information about the service provided (Example: adding modifier -80 indicates that the service is being billed for an assistant surgeon)

-HCPCS Modifiers may be attached to codes when they are billed in order to provide added information about the service/supply provided (Example: adding modifier -NU indicates that the equipment being sold is new)

-Place of Service Codes These codes indicate the location where a service was provided (Example: code #12 is used to denote that the service took place in the patient's private residence)

*The six CPT sections are:

Evaluation and Management

Anesthesia

Surgery

Pathology and Laboratory

Radiology

Medicine

(there are also Category II and III codes, but those are not categorized as "Sections")
